Person Centered Care Ltd is a Home Care Agency that has been in operation since 2012 and our mission is to continuously provide high-quality home health care services that empower individuals in their communities to live independently and with dignity.

Range of Services We Provide

Person Centered Care Ltd provides a range of services to Individuals with Health and Social Care needs living in the community. Our key focus is to support independence and choice for service users through a person centred approach. The agency provides a range of services to service users in Lincolnshire and Northamptonshire areas, from short visits per day to 24 hour packages. We provide service directly to individuals on a private basis or funded by Local Authorities and/or Continuing Health Care. Our carers provide a wide variety of services, as required, to meet the needs of individual customers. • assistance with washing/bathing and dressing
• 24 hour care
• Sleep-ins or working nights
• Light House Duties – meal preparation, laundry/housework etc
• Shopping, paying bills and budgeting finances
• Support with Medication 
• Daytime activities, access to community and social inclusion
• The pursuit of hobbies and interests
